RCAT™ - Root Cause Analysis Technique – LEADERS GUIDE

Designed for in-house training, this leader’s guide includes all you need to train your front-line supervisors in RCat™in a 4-6 hour session. Root Cause Analysis – RCat™ is a valuable tool to help an organization break the cycle of ineffective investigations and repeat accidents. It is designed to help an investigation team (or individuals) “cover all the bases” as they analyze the causes of the accident or incident. RCat™ makes use of two simple tools:

RCat™ Chart – a detailed chart/map linking incidents to their immediate, root (or underlying) causes, and the inadequate controls which led to the problem.

RCat™ Worksheet – designed to be used with the chart, the worksheet is used to record the results of the analysis and ensure that all causes are identified and that appropriate controls are recommended.

With these tools, investigators can be quickly trained to analyze accidents and near-miss incidents effectively. The end result is that they get beyond the symptoms and to the real, root causes of the problem.

The Root Cause Analysis Technique – Leader’s Guide contains lesson plans, a participant handout ready for reproduction, and b&w transparency masters to present a 4-6 hour workshop on RCat™. The Leader’s Guide also includes 10 Investigation forms, 10 RCat™ Charts and 10 RCat™ Worksheets Additional Charts and Worksheets may be purchased separately.
